This questionnaire is for completion and submission
by all suppliers who employ fewer than five people.
Completion of this form will be clearly marked on the
CHAS system and be viewable to all CHAS buyers.
Please note the Appendix 2 application should be completed by those who wish
to register with us as CDM Coordinator, Designer or Principal Contractor.
This form should not be completed by Care Service Providers – see Appendix 2B.
The person submitting this must be an employee of the business making the
application and have a sound understanding of your company's safety management system.
For this reason if our CHAS Assessor needs to talk about your application they will
liaise with you and not deal directly with any 3rd party consultants that may be
